What is the geographic distribution of visitation providers with respect to need? #157

Fiscal and providers are interested in knowing how well providers are meeting the need as described in #156.

For fiscal's interest, they are interested in better understanding where they could target resources for pockets of poor visitation coverage. They are also interested in monitoring mileage reimbursement.

For provider's, they are interested in predicting mileage reimbursement by estimating the mileage between their individual visit supervisors and the requested visitation service.

Regular reports to fiscal and providers could help meet both of these needs.
