Describe the bug
Very rarely on an approved visit report the name of the Visit Supervisor (which is almost always also the name of the Sprout user who entered the visit report in Sprout) gets populated on the visit report in the field for the Family and sometimes in the field for the social worker.
To Reproduce
Unsure how to reproduce this error. Here is an example. Screen shot attached from this visit report as well.
Expected behavior
You would expect the social worker's name from the referral to appear on all visit reports in the social worker field and the family name on the referral to appear on the visit reports as the family name.

Urgency or Concerns
This issue impacts billing, but a fix has already been deployed so that the names of the family, social worker are pulled from the referral and not from the visit report.
The far more concerning issue is that the visit report lists the wrong name for family and for social worker - and these visit reports are being downloaded and used in court and added to a parent's case file. So a fix should probably be prioritized.
