Describe the bug
DCYF user Bonnie White asked where to find the agency information and its contact information when a referral she made is picked up (this turns out to be a bug because there is no easy way to quickly find this information if one is using a Social Worker user account).
To Reproduce
Try to look at agency information and associated contact info when a referral has been made and picked up by a provider, from a Social Worker user account.
Expected behavior
A Social Worker user should easily be able to view this information without having to click through several screens/views.
