Users cannot see Date of New Referral on "New Referral" banner

Is your feature request related to a problem? Please describe. The user would like be able to see at glance when a referral dated.

Describe the solution you'd like Put referral date in the green notification bar.

Describe alternatives you've considered Train users to look at the authorized date in the bottom of the card, but this may not be the same date.
