Include new image map 66SWOLLEN_68TENDER_JOINT_COUNT #43

Hi Imagemap team,

As discussed with Marly I've mapped out a new image map for a 66 Swollen / 68 Tender Joint Map which can be used for scoring joint counts used in rheumatology research.

Can you take a look over the code changes and let me know if it looks ok for merging? When you're ready it would be really useful if I could test the live version of the image map to double check I've mapped and tagged the 136 checkboxes correctly.

Everything checks out, I'm going to merge and add your map to the Instrument Example.zip file to make it easily accessible.

@tlyn7657, release 1.5.0 of the imagemap module has the features in this PR and is now in the REDCap store. Sorry for the delays.
