unable to locate select2.css #32

https://github.com/ctsit/redcap_entity/blob/0e437c1003bbfe113126281320a52c521c2a8579/classes/EntityForm.php#L78 doesn't seem to resolve to a valid select2.css in REDCap 11.4.0. Not sure if it was merged into a global css file for REDCap and can be omitted or if it needs to be added to the EM and included directly.

Addressed by PR #37. The changes are in release 2.3.8 which has been submitted to the REDCap Repo.

@123andy, as I finished the 2.3.8 release I realized you are not in the authors.md file despite your PR #34. I updated that on the develop branch and it will go out in the next release. Sorry for the omission.
