Adding IP collection mode option

Review steps:

[ ] Go to the module configuration form and make sure you see a field named "IP collection mode" containing 2 options: "REDCap (default)" and "ipify"
[ ] Repeat testing steps from https://github.com/ctsit/survey_participant_ip/pull/1 for each available mode
[ ] Review README changes and make sure they make sense
