just about completed the information for the questionnaire which is in Google Drive at https://docs.google.com/document/d/19yxW-PguJsf7ctCs5AYUgOFQqPs50QxTcD5elVPIWq4/edit?usp=sharing

there are a couple pieces where I am in search of clarifications from @nleboeuftrujillo so can cover those in a future meeting. i linked the IR .doc file in the other google doc - have asked @sinkinsonc about the collection name to confirm. thanks!
