Still have some questions here.

Contact Form Fields for Wholeview Direct Resources Page:

1. I thought this was replacing the existing Contact Us form? Which page does it go on?
2. What should the form's title be?
3. Is there any other text etc. they want on the form?
4. Is it ok to use Jotform's design instead? Here is what I just made in Jotform: https://form.jotform.com/222806020135038

1. We need to swap this form out: https://wholeview.co/about-us/contact/ Same content.
2. Same title and blurb as on: https://wholeview.co/about-us/contact/
3. I'm fine with the form fields used here in the native form embed. We would have to remove the BG tho since it needs to fit in the page semi-elegantly https://wholeview.co/about-us/contact/
4. We also need a form on the Resources page: https://xd.adobe.com/view/a4a0e052-8ba9-4e95-85ef-8cccaccd2654-f1f3/screen/e1ff9e4b-ce04-4ce1-92cb-17a7716e90b2
5. The 'Quiz' on the resources page is for a later date.
6. The resources page needs to be coded and integrated.

@cunning-folk Just published the Jotform changes. The Contact Us form and the Wholeview Direct Resources page Jotforms are now live. (You or the client can edit the forms directly in your Jotform account, and changes will reflect on the form.) Let me know how they look.
