Insurance should be handled even with splits. Better to move insurance flag for dealing on hand basis.

Insurance was left in player. The rules are still unclear (after checking a few sites for rule clarification). I'll open a new issue for creating a rules option list. Apparently there are euro/american/montreal versions with even more variations of blackjack.
