ETD: Metadata audit of form and show view

ACCEPTANCE CRITERIA

• [x] Someone with administrator credentials should be able to fill in all of the metadata on the form page
• [x] When I save my ETD and go to the show page I should see the fields indicated in the metadata spreadsheet

Date fields should take a string and display it exactly as it was entered. No interpretation or formatting of date values expected.
