Support Subject Controlled Vocabularies #96

95 completed subject work, but did not implement the controlled vocabularies.

The spreadsheet asks for:

MeSH, AAT, FAST, LCNAF, local vocab/type ahead/ability to add and store with URIs

MeSH will be included in the initial scope - #124. Additional vocabularies may be added at the end of the project (time permitting) or as part of a future development phase.
