Use Hugo disqus template #39

This commit makes the following changes:

Remove the disqus partial present in the project
Use the Hugo provided disqus template to lessen maintenance, and provide a cohesive experience with the Hugo documentation
Update config files to not use the previous local disqus partial
Update the front-matter for the example posts

PLEASE NOTE that this branch is based off of master, but has changes in close proximity to pull request https://github.com/curttimson/hugo-theme-massively/pull/36 so it may need a rebase after one or the other are merged. (Simple conflicts, but conflicts none the less.)

@curttimson - Thanks for the other PR merge, and sorry for the delay (vacation). I've resolved the conflicts in this branch, and based it a bit more on your comments from the previous PR I submitted.

@shortword I've bumped the version to 4.0.0 because of the breaking changes and merged into master.

All appears to be working fine on the Netlify environment :)
