Hi Rowan,

My company has a need for a table of contents, and a footer with a page number in our document. I am also trying to get some title text above the table of contents. I am working to get these feature configurable. We’ll get a PR to you when we get this feature wrapped up.

We are thinking of something along the lines of the ability to pass in the docx objects for the table of contents, header and footer in the options object.
