Clean up LoginManager UI #122

Followup to #118.

Now that the structure of LoginManager is better we should clean up the user interface:

[ ] Get rid of spaghetti and replace by something new
[ ] Get rid of icon and replace by something new
[ ] When used through HMD with pointcloud representation, "invite" users to self-calibrate
[ ] Show that we're in auto-login, and where we are
[ ] Rationalize settings
