Add game start menu #251

The main menu is getting crowded. The different game start / mode items should be moved into a "Game start" sub-menu:

Careful with this one... with https://github.com/cxong/cdogs-sdl/commit/8dc652a49689c9491994b4f778882cf7e45c5758 the game will attempt to go back to the last game mode menu when finished. This streamlines deathmatch play, as players can quickly select the next map, but will complicate things with the game start menu.
