Updated Visual Studio Code Installation & README #9

I have migrated to newer version Visual Studio Code Extensions. Furthermore, I have added more clear installation instructions in a separate README for Visual Studio Code integration.

Finally, I added Table in the main README on supported features. I have not fully verified working and non-working all features in these LSP implementation, and the table may need to be updated.
