Updated Readme.md.
Add pulse count sensor to the sensor/toon_smartmeter.py script.
Updated sensor prefix from sensor/toon_smartmeter.py, since not everything will be from the P1 port.
Note for device id in sensor/toon_smartmeter.py, this id differs per type of meteradapter.
Fixed handling with invalid JSON output from newer TOON firmware in climate/toon.py.
