Don't show additional contact fields in addresses on registration, nor when there's a single address #305

Currently we show the additional contact fields in the address details, if the user can have additional contacts. However, when there's a single address this is confusing, because the user will see the mobile / landline / e-mail fields in 2 places, even with a single address. We should only show those fields if the user has more than 1 address. Also hide from the registration form.
