Add rescheduled option to appt outcome #3

Summary of functionality to add:
- Add “change” button to appt outcome like we do when confirming the babies.
- When changing appt, it shouldn’t show up that they’ve cancelled in the
study history.

What page (e.g. Baby Search) will be affected?
- Appointment Outcome

This is a difficult feature to add to appointment outcome. Instead, I have 
changed
"Change/Confirm Appointments" section with the following:
- When searching, you can now indicate that you want to search appointments 
that have
already been confirmed
- When you open up the specific baby form and appointment details, I have added 
a
form field allowing you to adjust the scheduling status between SCHEDULED and 
CONFIRMED.

confirming the baby using the form field works fine.

i can't quite see how to do the rescheduling though - maybe i am missing 
something. 
if you do a baby search, then click on their serial, the appointment details do 
not
come up at the bottom.

ok i've got it - i need to search them under confirm/change appointments.  this 
works
really well from what i can tell so far!
