Acabei de reproduzir os mesmos passos em PRD mas obtive sucesso.

Aconteceu só uma vez? Tinha algum proxy rodando ou algo do tipo? Esse IP 13.0.1.132 conhece ele?

Agora vi aqui, aparentemente essa função de recuperar a senha pelo metabase não deveria nem existir por isso o problema... @beatrizaantunes confirma isso?
