fix erroneous init_arg for prereq_os; add documentation #15

The use of the plugin name to hold the OS name is useful in simple cases, but can easily break down (e.g. when adding the plugin from a bundle or another plugin). Document the explicit use of the prereq_os option.

This fixes the issues described in #14.
