Make ODF backend feature complete #6

Before releasing to the public, we need to make sure that the ODT backend, for all asciidoc functionality, creates correct ODT output.

Currently we lack support for certain functionality, as well as some functionality fails to work. This issue is to track the remaining issues find with the ODF backend.

Currently the implementation is lacking:

TOC support
Table styles
Verse and Quote styles
Qanda styles
Glossary and Bibliography support
Example Blocks
Caption text
Numbered lists
{numbered}-macro not implemented
Appendix support

And probably some more...
