api,contract: remove ethIndexer, use eth_transfers

Replaces our aggregated balance-based ETH inbox.

This simplifies a lot of code. We handle ETH symmetrically with other tokens now.

Shipping once deployment succeeds.

• CREATE MATERIALIZED VIEW... took much longer on the mainnet dataset. Killed after 10 hours.
• Deleting irrelevant eth_transfers takes forever, currently running, ETA 1 day.
• Once that is done, CREATE MATERIALIZED VIEW should run quickly.
