Top/bottom padding should not collapse #12

louis:format has currently implemented top and bottom padding using liblouisutdml's linesBefore and linesAfter, which are collapsing. Liblouisutdml doesn't support non-collapsing margins.

Closing because I'm not planning to further develop liblouis-formatter.
