Remove M2e lifecycle-mapping (moved to module parent pom)

I fixed the POM file because it was reformatted by Eclipse again which made the diff unreadable. I fixed it. I also dropped the changes to dtbook-to-html because I think it was added by accident and should be removed (see https://github.com/daisy/pipeline-scripts/commit/28caef).
