dtbook-to-daisy3: insertions of text by px:fix-dtbook-structure #85

px:fix-dtbook-structure must be called before px:tts-for-dtbook, in case px:fix-dtbook-structure inserts text that should be synthesized.

In Daisy3 audio-only (option --with-text false), there will be validation issues, since the specs do not allow headings with no audio attached.
