I.  Issue Description: SQL installation errors when installing plugin_manager

II. Steps to reproduce the Issue:
1. install roundcube while using a custom sql database prefix
2. follow install guide for plugin_manager
3. agree to allow plugin_manager to alter sql
4. see errors stating sql database user doesnt have permissions (although it 
does)

III. Steps to fix the issue:
1. install roundcube while leaving custom database prefix empty
2. follow install guide for plugin_manager
3. agree to allow plugin_manager to alter sql
4. continue working with plugin_manger
