[calendar_plus] CalDAV calendar empty, no user/password settings #719

I.  Issue Description:
Under calendar sources, I added my caldav server URL (which works fine with 
Thunderbird, iPhone and Android phone). BUT, nothing happens... My calendar is 
empty in Roundcube.
Note I've only one parameter in the calendar_plus server, not chance to set 
user/password (which are required to access the calendar).

II. Steps to reproduce the Issue:
1. Add a "calendar sources" (I use French version) with correct URL
2. Go to Calendar, it's empty

First of all update your Roundcube and plugins. Roundcube 0.9.5 is outdated.
Second this is not a bug report, it is a support request.
Goto http://forum.myroundcube.com

The plugin is known to work if ...
a) using recent versions
b) configuration is done as excepted
