I know this Add-On is only intended to be used within a native Google Doc, but I think this may explain some of the other reports for the add-on not installing or being available. #13 and #8 . I ran into this myself and realized I just wasn't editing the document in the correct format.
If you upload or are sent an e.g. MS Word document, and edit the document in Google Docs, the SimpleSignature Add-On is does not appear. In my case, it is the only add-on I had installed, so the Extensions menu did not appear at all after I installed it. Converting the document from .DOCX format to a native Google Doc made the add-on appear and function correctly.
It's not obvious from a user perspective because there was no feedback from the add-on itself or the extensions framework specifically indicating that the add-on could not be used, or that the document type was not compatible.
When I installed the add-on in the .DOCX document, there was a 2-step installation dialog. The first said the installation completed, and showed a picture of where to access extensions from the menu. The second dialog had the following message:
