Very good writeup. A couple of points not specifically mentioned that might help someone: First, make sure you install the 32 bit version of Perl, as the 64 bit version requires more complex workarounds to work with IIS. Second, the ActiveState install includes a utility to configure Perl with IIS automatically...
C:\perl\bin\ap-iis-config.bat
Use the option to install for all sites and save a lot of trouble
