Request: add/configure Issues (and PR) templates #20

Nothing too fancy, just some basic fields for some consistency.

I'm putting this in a milestone. No longer a Request - I'm actually going to have a way for this to work. @danbush I'll handle the github issue part, I'll just hit you up for some github owner stuff at some point.
