bug fixes, doc updates, some code changes too #3

hi daniel - had some time tonight while babysitting a few vm clones so i thought i'd update the git repository. still haven't worked on the test suite mods, soon i hope. i added some docs regarding negotiated / reduced rates and some other minor code changes i made on my side but were not in the repository. also added indonesia as a country to config.ini file.

Awesome, thanks! I've got a little bit of cleanup to do on the patches. For example, I'm not sure it's a good idea to have separate elements for from_state and from_state_abbrev rather than calculate one from the other. But

I'll take a closer look.
