Add dropdown lists for Provinsi, Kabupaten and Kecamatan for all of Central Java #43

Use javascript to change the dropdown options based on province... then kabupaten, then kecamatan. Include kecamatan for all of Central Java, Yogya and Jakarta. This will improve the quality of data entered and improve the accuracy of generated reports / stats.
