I think it would be fun to add neutral unreliable NPCs to the levels. They can run around, scream and run away when they see the player (attracting monsters) and be easily killed by monsters (should not help player by distracting monsters too much).
This could add some room for storytelling (maybe they scream some facts about the world or monsters or have little camping sites with personal belongings with lore attached to them) as well as give the world some lived-in-aesthetic.
We probably don't need it for 1.0 since it could require some work coding the AI.
