(win)fixes #96

A few fixes. One commit is specific for Windows platform, which needs to define PD_INTERNAL; it closes #95. The other one is simply clearing a leftover.

This is now longer needed as PD_INTERNAL is now required for building libpd 0.14 and I have it set now for all platforms in the latest commits. Also, you removed HAVE_LIBDL which disables loading precompiled external objects.
