ConceptIntersect - allow for records outside of observation to be included #710

Would, I think, be nice to have an option to include records outside of observation (if inside the specified window)

that would go against the "observation_period" definition, but we can consider... Which would be the use-case?

To replicate particular atlas cohorts, the cohort would be in observation it could just potentially use records out of observation (but wouldn't by default)
