CSV Output #40

Added initial CSV output. This now outputs two files

Error Summary with Error Code, Error Description and Total # of errors
Child-level error summary with Child ID, Error Code, Error Description, and affected fields

This is achieved by a new button - and some of the button spacing was changed to be simpler as well.
