[rig eval] fix some bugs

• move the option “Apply the same evaluation to the next item” [to the bottom of the questions]
• Remove toolbar
• update Q4 to Query 4
• update footnote text font styling
• Move the “Re-eval” button to the left of the “List button”
• update highlight colors
