sv index updates to handle global mortality/death rates

• remove global from topic descriptions
• mortality rate/death rate -> mortalities/deaths
• remove "mortality rate" and "death rate" from Per Capita exclusions because the exclusion list is all forms of "x rate" that shows up as sv descriptions in the index

Why does dc/topic/GlobalHealth come before dc/topic/Health? Is it the capital H vs. small h?

As we had talked about offline, updated GlobalHealth descriptions to have the same capitalization as Health and HealthConditions.
