Replace links to GitHub Pages with Netlify README #487

The README links to the demo catalog instance in several places, mostly to the schema view. Since the demo moved from GitHub Pages to Netlify (#421), these links needed to be updated.

Indeed, there's a bunch of other necessary fixes that would make sense to include in this PR too, thanks for linking the new issue.
