Document steps to clean up a sunset web application

As we're moving more sites to Heroku, the list of old and inactive sites on our servers is growing. We should have a documented process for the steps to take to remove and cleanup these unused web applications.

Steps if the site is on its own server:

shut down the EC2 instance

Steps if the site is on a shared server:

stop and remove the supervisor process
delete the supervisor and nginx configs
remove the ssl cert
delete the app directory
drop the database
remove the virtual env
