There are countries without country code (e.g. Kosovo)

This PR considers that the default is "required=true", as discussed in https://github.com/dataprotocols/dataprotocols/issues/167. Although it's not part of the spec yet, jsontableschema uses it.

It seems that required will default to false, in which case this PR isn't necessary anymore.
