Rename this to datasette-query-assistant #7

I may want to use datasette-assistant as the name for something else in the future (the mythical "AI data assistant"). datasette-query-assistant or datasette-sql-assistant would be OK for this, I like the former.

A reason-query-assistant beats -sql-assistant is that this plugin may well grow to sometimes send users to a table page with ?filters rather than a SQL query in some cases.
