Set `suggested-transport-medium` to `webform` for a few companies

We only store location data associated with a mobile device ID, so I don't have any way to look up your information unless you provide a device ID for us to look up. It more easily handles these requests so there isn't even a paper trail left behind, and it also has instructions on how to find your mobile device ID.
