Make orthographies work #290

See the Kwak'wala OLD as a test case since it has complicated orthographic mappings.

One issue is that it uses XML markup for superscripts in some orthographies and this is not supported by the current OrthographyView interface, i.e., "dz" is being rendered as "dz".
