Move Chat channel from gitter to discord

Chat channel is important and still quite actively used: https://gitter.im/datahubio/chat

Gitter is getting a bit old and discord seems good so let's move there.

Acceptance

• [x] New server and channel exists on discord See https://discord.gg/KrRzMKU
• [x] Notified in existing gitter channel DONE on 21 June
• [x] Changed on datahub.io website (with nice icon) @sglavoie -- DONE: See this comment confirming that changes are live on the website.

Analysis

? should we create a dedicated server or run under datopian server? (You can't move channels to another server atm on discord ...)

KISS (and promoting one thing) => put it on the one channel

FUTURE / one place for one thing => put it on its own server ...

I think on reflection it's worth its own server ...

• The code has been committed locally and is ready to publish. However, because I need to push to the master branch to trigger the build and be able to see it in the staging environment, I would like to confirm with you if you like the changes first as to avoid cluttering the Git history.
• All links were updated to point to the Discord server instead of Gitter.
• Additionally, I added a separate commit to include an icon for GitHub because it is being displayed in the same footer as Discord and I thought it looked better that way to maintain consistency in the way links are presented.
• Those images were also integrated as emojis and can be used in posts with :discord: and :github:.

• There was a server issue impeding the deployment, but all is fixed now!
