Banners - Remove Text/Graphical option

When uploading banners, users are presented with an option for it to be 
categorized as a text or graphical banner.  This is not needed any more.

We should take out the code that presents the users with this option.  We 
should also file all banners under one default category.  I'm guessing this 
will have to be "/graphical/" to avoid recoding a lot of stuff.

There are currently only 9 banners uploaded as "text banners".  These should be 
re-uploaded as graphical banners and then the text ones should be removed from 
the site.
