for-later still has kthread imported in visorhba #121

With Tim's changes to kthread it caused a conflict with the for-later branch. I've resolved the conflicts but did notice that we are still importing kthread and and still had threadinfo in the device structure. We shouldn't be using those anymore in for-later. I believe it needs to be cleaned up and squashed.

Changes addressed in branch visorhba_remove_kthread need not be rebased or modified in any way. Changes have already shown up in for-later by virtue of other work. Closing...
