Change generation of case_tube_idx #42

Goal is to "freeze" file_list. We want case_tube_idx to be ordered based on file_list (rather than being dynamically generated in the database), so that regeneration of the meta_db will not accidentally change the case_tube_idx to Case_number/case_tube mapping.

case_tube_idx is now consistent. It is derived from file list.
